Voyager 64Gb stopped working after 1 month without using it.

In both Windows XP and Linux symptoms are the same:

1. Plug in

2. Os starts to seach the drive

3. Os finds it

4. Drive disconnects

5. loop from 2 to 4 starts to happen.

 

Part was RMA part for broken part and it was working fine but after I did not use it for a month the above problem appeared. I tried instructions from http://forum.corsair.com/forums/showthread.php?t=62742 without success. I managed to delete the partition from stick using the instructions but creating partition from fdisk resulted stick disconnecting while fdisk was checking the partition (did not even pass 0%).

 

Diagnostics from Linux kernel (2.6.29.4) say before fdisk operation:

The serial number of my part can be found from logs and it is 64Gb Flash Voyager. This is the second time that stick has broken without false usage... Is there any way to salvage the part?
