Bad VS1GSDS667D2 stick

I recently installed 2x VS1GSDS667D2 sticks in my Mac mini, and the machine began having sporadic kernel panics (OS X crashes, displaying the "You need to restart your computer" message). I ran the Apple Hardware Test, and after a few minutes of repeating the test (in loop mode) it failed with the error "4MEM/4/40000000," indicating bad memory. I installed the memory in another machine, which subsequently failed the hardware test with a similar error.

 

I was able to isolate which stick was causing the problems, by installing only one stick at a time and repeating the hardware test. How would I go about getting a replacement stick?
