Bad Module, 50,000 errors - Corsair ValueSelect 512MB (VS512MB400)

Owned two of these modules since Christmas 2004. One still working perfectly, second has failed. With only the faulty module installed when I boot windows I get hundereds of memory errors "the memory could not be 'read' at address 0x######" etc, and when the module is removed, and the other working module is installed, the errors do not occur. I ran Memtext86 having installed only the faulty module, and within the first 10 minutes racked up around 50,000 errors:

 

Test 0: 0

Test 1: 313

Test 2: 3607

Test 3: 19579

Test 4: 9338

Test 5: 965

Test 6: 11731

Test 7: 6004

 

The memory address range which kept showing errors was:

00000dba#### to

00000dbb####

and was apparently 13.6MB(?)

 

I have also tested this module on another computer and had exactly the same results.

 

Please advise, is this module dead?, and if so, can I obtain a RMA number for it?
