Reporting a broken module

I want to report a broken module VS512MB400 Lot 0345077. It used to work, but my PC started to crash and after testing found it was this module. The module was purchased 15/11/03 and is now out of supplier warranty.

 

Please help.

 

When returning this module, is there an address in the UK I can send it to?

Can you tell me the make and model of MB you have along with the CPU speed and it’s FSB as well? In addition, please tell me the bios settings you have set for both CPU and memory and any performance settings that you may have set?

This is one of a pair I bought from Scan.co.uk last year. The module worked for about 9 months and then went wrong. The second module is still working properly.

 

The motherboard is an abit IC7-MAX3. CPU is 2.8 at 800mhz. No special bios/performance settings - all default.
